Job Title: Home & Community Nurse

Job Description:

Conduct hospital or home visits to assess new clients for home care services or when required.

Ensure strict compliance to documentation as required by MOH for financial and quality audit.

Lead relevant projects for the development of our services.
-
Train all new care staff joining Active Global to our standard of care and other relevant trainings.

Work alongside external community partners and colleagues in managing client care needs.
-
Conduct mediation / counselling sessions between care staff and clients when needed.
-
Advise care staff as needed if they face medical issues or medical emergencies with clients.

Working together as a team to handle complaints and crisis management.
-
Any other adhoc tasks assigned by the management.
Requirements:

Locals Only
-
Nitec, Diploma or Degree in Nursing.

Must be a Enrolled or Registered Nurse with Singapore Nursing Board with a clean disciplinary record (nurses on conditional license may not apply for this role).

As you will be working as a team, some weekends including public holidays may be required.

Proficient in Microsoft Office and comfortable with IT.
